Best way to approach template design?

I want to design a web template for a friend and wanted to get your opinion on the best way to approach template design? Which programs should I use? I know quite a bit but still am not knowledgeable enough to use notepad not yet atleast :) so what do you think I should use?

1. Get a free program like Netscape Composer. Make a large table with no borders, cellspacing, or cellpadding. Use a paint program to make images that fit exactly into the cell widths, or just use bg colors. Use a nested table to achieve padding where needed.
